Fill in your Registration Details.
Make Payment of Rs. 500 for Patient Registration. Scan the QR code and make the payment. You can also make payment through our UPI ID: drprabhat54@okhdfcbank or through Phone number 9434198583.
After successful payment of Rs. 500 you will be PROVIDED WITH LINK OF CASE RECORD FORM. After opening the link read the CASE RECORD FORM carefully and then fill the CASE TAKING FORM in detail as far as possible.

Online consultation fees:

We offer 4 different packages to suit your need, so that you can avail the most cost-effective treatment based on your requirement.

One month treatment plan = Rs 2,000 only
Two months Treatment plan = Rs 3500 only
Four Months treatment Plan = Rs 6,500 only
Six Months Treatment Plan = Rs 8500 only

Constitutional main course medicines will be delivered for the chosen period. If rarely any other extra / accessory medicine is essentially required, prescription will be served what is to be purchased by you from medical store.

If the payment as per your choice of package is deposited successfully, then you will get a call from Dr.Bhattacheryay's clinic and Reg. No of your Case will be forwarded to you. You will then be advised to send the filled in form through email (drprabhat54@gmail.com) or whatsapp (+91-9434198583) mentioning Case Reg. No. over Form. Please send the Filled in Case Taking Form along with diagnostic reports through email (drprabhat54@gmail.com) or whatsapp (+91-9434198583).
After submitting Form and sending reports via whatsapp, you are requested to give a call to 9434198583. Doctor then will go through your case record and reports. If required he may require a phone call or video call to you for getting further information, if any within two days of receipt.
Give your detail and precise postal address with Pin Code and Land mark through whatsapp. Medicine with instruction will be dispatched at your doorstep shortly. Dispatch details will be sent to you.
